Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, healthcare providers and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

Frequently Asked Questions

Do you accept insurance?

I currently accept Aetna, Anthem, and United/Optum. I have partnered with Grow Therapy to make it easier for clients using insurance to utilize our services. Please use this link to book an appointment if you are using one of our accepted insurances: https://growtherapy.com/book-appointment?id=35692&utm_source=provider-sourced&utm_medium=booking-link&utm_campaign=provider-dashboard

If I have insurance, but choose to pay out of pocket, is that possible?

Yes. Some clients opt for private pay instead of using their insurance, for reasons including privacy and flexibility. Insurance companies require a clinical diagnosis, dates visited, and may limit the number of session visits based on diagnosis. Full payment, or copay for accepted insurances, is due at the time of service. I accept cash, check, credit card, flexible spending or health savings account cards.

What if I have insurance from a provider other than those listed?

For all other insurances, I am an out-of-network provider and will provide a “superbill” upon request to submit to insurance plans for reimbursement. Many insurance companies reimburse for a portion of out of pocket expenses. Please contact your insurance company to inquire:

  1. Does my coverage include mental health benefits?
  2. What are my out of network reimbursement rates for a mental health therapist?
  3. If the insurance requests CPT codes, the most common are 90791 (intake) and 90834 (individual therapy).
  4. Do I have an out of network deductible and have I met it?

Where is your office located?

The office is located in Belle Haven, Alexandria, a park-like setting approximately two miles from Old Town and convenient to GW Parkway. Parking on the street and in the well-lit lot is free and plentiful. Our address is: 1707 Belle View Boulevard, #A2, Alexandria VA 22307.

Our unit has its own entrance, marked clearly in front with an A2. When facing the front of the 1707 building, our separate entrance is around the corner to your right. Please have a seat in the waiting room and I will greet you at your appointment time.

Please note that although the office is on the first floor, it is unfortunately not accessible to those who cannot walk up and down porch steps. Virtual appointments are available as an option.

Do you offer a free consultation?

Yes, I offer a free 20 minute phone or virtual call to answer any questions you may have and to see what it would be like to work together. Please email me directly at sgolub@wtenova.com or use the contact form to schedule the free consultation.

Do you see clients in-person or virtually?

I offer both in person visits and virtual appointments. I highly recommend an in person visit for the first appointment and intake if possible.

What can I expect in the first session?

Our first session will be spent on reviewing your history as well as what brought you to therapy. You are the expert on your life and I will meet you where you are at and then partner with you step by step to establish your goals. I will also answer any questions you may have and together we will start working on creating a treatment plan tailored to your needs. My hope is that you will feel supported and empowered to accomplish more than you ever thought possible, as soon as our very first session together.

Do you see men?

Yes! Although I specialize in women’s issues, I have years of clinical experience working with men and welcome men whose concerns are within my areas of specialty.

Let's start your journey

I will tailor your treatment to your individual needs. I look forward to learning more about you and partnering with you on this important and life-changing journey together.
